Instructions for transfers by certified mail - If you hold securities in certificate form,
you will need to mail two envelopes separately to complete your gift. In the first envelope,
place the unsigned stock certificate(s). In the other envelope, include a signed stock power
for each certificate. We can email the stock power form to you. Please sign the form and have
your signature guaranteed. A signature guarantee can be obtained at your broker or a bank
branch. If you are not able to get a signature guarantee we can confirm your signature
and ID via video conference.
Please note that NO information on the stock certificate or stock power should be
completed except the date and signature fields. The stock certificate and the stock power
can be placed in two separate envelopes and sent certified mail to:

Please note: This notification form is not a receipt for your charitable donation. We will send you an acknowledgement after we have received your gift. The value of a charitable gift of stock is determined by the average between the high and low stock price on the date of the gift. Mutual fund shares are valued using the closing price for the fund on the date of the gift. The date of the gift is the date the charitable organization receives the asset. Consult your broker for more information on the timing of the transfer.
